Record Keeping Requirements
Whenever a person volunteers for the department, the division/office must complete and keep on file the Personal Services/Volunteer Personal Data Sheet (DNR1234). The Motor Vehicle Accident Liability Insurance Coverage Form (DNR1233) should be completed and submitted to the Division of Real Estate and Land Management with a copy retained in the division/office.
Workers Compensation Coverage
The department has contracted with the Bureau of Workers Compensation coverage for volunteers. In the event of an accident, please notify the Office of Human Resources (OHR) immediately and forward to OHR a copy of the Personal Services/Volunteer Personal Data Sheet (DNR1234) that was completed on the injured employee on the date of hire. OHR will contact the Bureau of Workers Compensation.
