County and Township dam is in:	If dam is in more than one county or township, list both.
Description of the dams location:	Example: DAM--at the Lima Reservoir station downstream about 3/4 mile of the Lima Reservoir down from Fetter Road bridge in Lima
Latitude and Longitude:	If you have a GPS unit, please record the lat. long. of the dam.
